This cost will not be processed through insurance.
All medical clearances related to cosmetic surgery procedures including,
but not limited to
breast reduction, abdominoplasty, BBLs,
liposuction etc.
Will be billed as self pay.

FMLA / DISABILITY PAPERWORK ($150)
This paperwork will be completed once payment has been received. Additionally, no prior authorization will be obtained for prescriptions that are not covered by insurance.
NO-SHOW / SAME DAY CANCELLATION ($50)
Make sure to call the office 24 hours prior to your appointment time in order to avoid incurring any fees. If you are calling after hours, please follow the prompts and leave a voicemail requesting a cancellation.

This is solely for the blood draw service and does not include the cost of labs.

Recording of any kind including but not limited to video, audio, live transmission, or other means is strictly prohibited. You need to get consent from the concerned staff in order to do so.
