SUPPLEMENTARY DOCUMENTS & PAPERWORK

All supplementary documents and paperwork will be completed and signed at the discretion of the primary care doctor / medical office.

The primary care doctor / the medical office retains the right to not complete supplementary documents and other paperwork that they do not think necessary in their professional judgment.

Before any supplementary documents and other paperwork is completed and signed, the appropriate appointments will need to be completed and the relevant fees need to be paid.

Some supplementary documents and other paperwork will require the patient to complete an office visit, complete labs, and then come to the follow-up visit to discuss the results and complete the paperwork.

Please allow at least 10 business days for any paperwork that needs to be completed to allow for necessary appointments to be made.

FEES

1. $250 per document
2. $150 FMLA / DISABILITY PAPERWORK
3. $100 per document for expediting (3 business days or less)
